Responsibilities:
Case Managers are a key component of the SSG Fox Grant Program team responsible for engaging with Veterans, service members, and military families.
This is achieved through community linkages with resources on a local, regional, and national level and coordination with SSG Fox Peer Support Specialists assigned to the same geographic area.
The Case Manager is responsible for maintaining a Veterans First mindset, maintaining accurate records and data integrity through a Salesforce-hosted database system, delivering quality and timely service, and continuously ensuring a high level of knowledge regarding potential resources.
Specific responsibilities include, but are not limited to:
Collaborate with peers and community partners to ensure seamless service to Veterans, service members, and military families.
Maintain extensive awareness and knowledge of resources available for Veterans, service members, and military families as well as general community resources within their assigned geographic area.
Perform participant needs assessments and screening for the SSG Fox Peer Support Grant Program and ensuring that all VA required eligibility screening is completed in accordance with regulations.
Continuously screen for possible housing assistance and/or homelessness prevention, and ensure referral to appropriate resources within the local COC (Continuum of Care) including local SSVF and HUD-VASH providers.
Creation of individual, integrated care plans.
Developing client-guided, actionable, timely, and reasonable goals intended to address the participant's obstacles to obtaining services.
Conduct quality control to ensure quality service and data integrity.
Continuously monitor personal workload and quality of notes.
All other duties as assigned.
Qualifications/Knowledge/Skills:
Enthusiastic about agency's mission.
Attention to detail is critical.
Extensive Military/Veteran cultural competency.
Ability to work with diverse populations.
Proven ability to continuously deal with mentally and/or emotionally challenging situations.
Experience in dealing with resources in the community.
Extensive experience with modern office IT systems, including Microsoft Office suite - computer literacy is an absolute must.
Salesforce or other database / CRM system familiarity a plus.
Excellent communication (written and verbal), interpersonal, and organizational skills.
The ability to multitask and prioritize workload in a fast paced environment is absolutely vital to this position.
Complete mandatory background check, training, and 90-day probationary period.
All other duties as required.
Interviews will be held the week of April 25th, with an expected start date of May 8th - details to follow for successful applicants.
Candidate should be prepared to attend training in our Buffalo office the first few days of employment, and work remotely until the office in Mount Morris is available for occupancy - tentatively June of 2023, subject to change.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
