Responsibilities

Plan, manages and evaluates the care management training program, Care Management Skills for Interdisciplinary Professional Clinicians. Develops new and modifies existing curriculum and training materials to ensure all care management training and educational needs are met. Recommends and implements approved changes to ensure the program meets standards of practice. Works under general direction.

responsibilities

Manages and oversees all aspects of care management training program including dissemination of program materials, oversight of on-line training sessions, and in-person training sessions.

Conducts live training sessions and interacts with trainees to ensure understanding of all care management competencies. Provides additional in-services, training sessions and other learning activities as needed

Ensures care management training program is aligned with the goals of the Workforce Development Initiative (WDI) including value based payment measures.

Manages and oversees enrollment in the care management training program and ensures that trainees complete the course in timely manner.

Ensures that all staff enrolled in the care management training program have access to training materials and online training modules. Supports trainees in all aspects of the training program to ensure successfull completion of the course.

Ensures the care management training program is in accordance with standards of practice in care management, VNSNY care management policies and procedures and aligns with the exam requirements of the Commission for Case Managers (CCM) certification exam and American Nurses Credentialing Center (ANCC) certification exam.

Reviews care management training program outcomes and program evaluations to determine areas for improvement; makes recommendations for changes and/or new programs for care management as appropriate. Presents improvement strategies to department leadership. Participates in development of revised program and/or materials.

Designs new and modifies existing educational curriculum and training materials to ensure staff has current knowledge and competencies to take care management exams. Ensures that training curriculum is updated continually to meet changes in standards of practice or exam competencies.

Works closely with the Quality department in the development and the implementation of training programs that focus on evidence-based practice.

Manages Web-Ex training modules and ensures that all training modules are presented in a clear and effective manner.

Ensures program content is presented in a manner that promotes learning and enables trainees to meet competency and exam requirements.

Utilizes resources and practices sound fiscal responsibility. Monitors expenditures and ensures compliance with budgetary constraints.

Assists with compiling and disseminating training course materials and other administrative responsibilities such as room scheduling, tracking trainees, exam dates, etc.

Demonstrates service excellence and creates and implements care management training programs which are centered on the goals and strategies of the WIO.

Participates in special projects, serves on committees and performs other duties, as required.

qualifications

Licensure: Current license and registration to practice as a Registered Professional Nurse in New York State required.

Education: Bachelor s or Master s degree in nursing required. Certification in Nursing Education preferred.

Experience: Minimum four years experience in a community based or long term home health setting, including at least two years with a primary focus on education and training, required. Experience working as a case/care manager preferred. Knowledge of health care delivery systems, patient care, care coordination, and clinical processes required. Effective oral and written communication, presentation, analytical, coaching and organizational skills required. Knowledge of computer applications required.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
